We require you to sign and return the terms and conditions, and also to provide the technical set-up details we have asked for. Once we have received these, we can send you the link to your new FormularyComplete.

I don’t know the answers to some of the questions in the survey
You will need to liase with your IT colleagues/department for questions 5 and 6. Please do this to ensure we have the correct information to allow your access to FormularyComplete.
How do I find out our IP ranges (question 5 of the technical survey)?
You will need to liaise with your IT department for these, and you must ensure that all the ranges you provide us with are static, externally facing IP addresses. You will also need to check whether you are on the internal NHS network, known as the N3 Gateway – this will mean you will have to access FormularyComplete via a referrer URL (see next question for further information regarding this).
How do I find out our referrer URL (question 6 of the technical survey)?
The referrer URL has to be the exact page on your intranet that will contain the link to FormularyComplete. If you don't have the link on the designated intranet page, access won’t work. We would suggest working with your IT department to determine the most suitable page.
